Help Shape the Future of Community-Based Primary Care Under the direction of licensed healthcare providers and clinic leadership, the Licensed Practical Nurse (LPN) delivers high-quality, patient-centered care in a primary care clinic setting. The LPN plays a vital role in supporting clinical operations by conducting patient intake and assessments, preparing patients for examinations and procedures, administering medications and immunizations, performing routine diagnostic testing, and assisting providers in the delivery of safe and effective care. The LPN supports care coordination and patient engagement activities through patient education, telephone triage, follow-up communications, and collaboration with patients, providers, and community healthcare partners. The position is responsible for maintaining accurate and timely documentation in the electronic health record (EHR) and ensuring compliance with clinical, regulatory, and organizational standards. In addition to direct patient care responsibilities, the LPN assists with clinic operations by managing medical supplies and inventories, maintaining examination rooms, supporting vaccine management activities, and performing related administrative functions. The role contributes to quality improvement initiatives, EHR optimization efforts, workflow enhancements, and other projects designed to improve patient outcomes, operational efficiency, and regulatory compliance. The ideal candidate demonstrates strong clinical skills, excellent communication and customer service abilities, and a commitment to collaborative, team-based care. This position requires a compassionate approach to serving diverse patient populations and a dedication to advancing the mission of the clinic through excellence in patient care and continuous improvement. Direct Patient Care and Clinical Support
The clinic is open Monday through Friday from 8:00 a.m. to 8:30 p.m., Saturday from 9:00 a.m. to 5:00 p.m., and Sunday from 1:00 p.m. to 5:00 p.m. This position requires flexibility to work assigned shifts during the clinic's operating hours, including days, evenings, and weekends. Employees are expected to work 40 hours per week. Work schedules may vary and are subject to adjustment based on patient care needs, clinic operations, staffing requirements, and coverage needs. Candidates must be willing to participate in schedule changes, shift exchanges, and other staffing arrangements necessary to support continuous, high-quality patient care and efficient clinic operations.

Licensed as a practical nurse in accordance with Chapter 464, Florida Statutes. |

At least 2 years of experience preferably in ambulatory care and/or primary care setting Experience working in a rural health clinic setting
