Job Description:

The Department of Medicine, Division of Hematology and Oncology is seeking a full time Registered Nurse II. This position is a member of the UF Hemophilia Treatment Center support team. The UF HTC is funded primarily by a HRSA-approved 340b pharmacy program, the income from which is required to directly support UF HTC patients, defined as all hemophilia patients treated by UF physicians. The Center encompasses 4 faculty and 5 staff, with $1.9 million in revenues. The position provides direct patient care to all HTC patients, focusing on clinical care between provider visits, including prescription refills, insurance prior authorizations and other medical supply needs. The position reports to both the HTC Medical Directors and the Department of Medicine’s Associate Director for Clinical Operations. The incumbent is responsible for maintaining sensitive and confidential materials that are received and/or developed during the course of the incumbent's job duties.

Essential Functions;

Direct Patient Support for Adult Hemophilia Patients

Provide direct support for UF-HTC patients and their families, by participating in team evaluations and the development of care plans during clinic visits, and by providing consistent follow-up with patients to ensure compliance with programmatic requirements.
Responsible for submission and tracking of all required insurance prior authorizations for medications and other medical therapies as ordered by the HTC physicians. The incumbent will provide this care in-person and via phone/secure EMR messaging.

Direct Patient Support for Pediatric Hemophilia Patients

Provide direct support for UF-HTC patients and their families, by participating in team evaluations and the development of care plans during clinic visits, and by providing consistent follow-up with patients to ensure compliance with programmatic requirements.

Proactive Patient Monitoring/Quality Assurance
Provide comprehensive and proactive monitoring of programmatic quality metrics at the individual patient level. Initiate care plans to increase compliance with said metrics, and report results to HTC Medical Directors monthly, and as needed. Work with the HTC Clinical Case Manager to coordinate necessary wellness visits, follow-up care, and/or supportive services when the need is identified.


Miscellaneous Duties

Prioritizes assignments and messages based on urgency and/or deadlines and works cooperatively with other Divisions, Departments, and other University staff to ensure that all assignments are completed in a timely manner and meet or exceed the standards for quality and performance as established by the Associate Director.

Serves as a liaison to multiple internal and external entities, including but not limited to Department of Medicine and all Divisions, College of Medicine, UF Health, VA Medical Center, referring physicians, patients, and patients’ families

Adheres to all office policies, procedures, and protocols as developed by the Center Medical Directors and the Department of Medicine Associate Director for Clinical Operations

Expected Salary:

$55,000 - $72,000 Annually

Minimum Requirements:

Graduate of a nationally accredited nursing program and one year of professional nursing experience. Licensed as a registered nurse in accordance with Chapter 464, Florida Statutes.

Preferred Qualifications:

Bachelor’s degree preferred.
• Experience in case management or Hematology in a large clinical setting.
• Experience with care plan creation and implementation
• Knowledge of and ability to effectively utilize the EPIC EMR Knowledge of and ability to use spreadsheet, word processing, and database
• Proficiency in Word, Excel, PowerPoint, E-mail and Internet
• Ability to plan, organize and coordinate all work assignments.
• Ability to establish and maintain effective working relationships with others.
• Ability to function under stressful circumstances.
• Ability to communicate effectively both verbally and in writing. Ability to plan, organize and coordinate work assignments
• Ability to work independently
