Job Description:

This position is responsible for outpatient case management and service coordination for children made eligible for Early Steps program. They will be required to make home visits for initial contacts to meet with parents/guardians to explain our program philosophy, eligibility and evaluation process, document the families’ concerns, update patient and family information, provide parent education on developmental issues and complete a developmental screening when necessary. This requires traveling throughout the designated service area. This position is responsible for attending all assigned clinics and evaluations both in- and out-of-town. The position is also the team leader for the Individualized Family Support Plan (IFSP) team, and is responsible for writing the IFSP with the family and the IFSP team. This position is responsible for implementing all services listed on the IFSP, completing the follow-up on those services to ensure quality and family satisfaction and completing state and federal mandated updates on the plan. The services on the IFSP must be in place within a state specified timeline. Implementing services on the IFSP requires having a broad knowledge base of the Early Steps provider referral system.

This position is responsible for determining funding sources for services. It requires knowledge of the Florida State Medicaid Managed Care system, Kid Care, and private insurance regulations and procedures, and the ability to ensure that services implemented under Part Care provided in the way the law stipulates. This position is responsible for completing documentation for service provision, maintenance of patient records, billing, and data collection. In addition to the documentation, this position enters the data related to the above into the statewide data system, which includes billing targeted case management activities in the state data system within the required timeline. This requires knowledge of the data system, documentation procedures, Medicaid documentation and billing requirements, and the regulations governing Florida's implementation of Part C.

This position is responsible for the timely and smooth transition of the children out of the Part C program to an appropriate program in the community when the child is no longer eligible for our program. The state now requires that this position attend all transition meetings, public school staffing, and individual education plan meetings for all children exiting the program.

This position acts as a community liaison for the Early Steps Program, networking with other providers of early intervention services, including the public school systems, private providers, and the medical system. This requires attendance at interagency council meetings, agency staffing, state meetings, and in-service trainings.

This position is required to participate in in-service training programs as specified by the State Early Steps Program. Completion of this requirement must be documented in the coordinator's personnel file.

Expected Salary:

$39,700 - $43,000 annually

Minimum Requirements:

Master’s degree in an appropriate area; or a bachelor’s degree in an appropriate area and two years of relevant experience.

Preferred Qualifications:
  • Bachelor’s degree and two years of experience including one year providing pediatric case management and/or social work.
  • Master’s degree preferred.
  • Knowledge of development, both typical and atypical
  • Knowledge of services for children birth to three years old, particularly with disabilities, strongly desired.
  • Computer skills strongly desired.
