Job Description:
Prepares and submits required monthly data reports, quarterly staff training logs, and quarterly community training logs according to contract requirements. Reviews community therapist monthly workbooks and billing. Approves and submits to Finance group for VOCA invoicing.
Functions as the Records custodian for the program. Retains client records in secured location and assists with retrieving records at the request of the Program Coordinator. Assists with the completion of intake documentation with clients, caregivers, and/or referral sources to obtain demographic and pertinent clinical information prior to the evaluation of pending clients. Serves as point of contact for the office. Assists with maintenance of client waitlist as outlined with waitlist policies and provides regular updates to the Program Coordinator.
Serves as the Community liaison for the program, building relationships within the community and the clients that we serve. Finds opportunities for us to exhibit our program at community events, meet with other community partners and train the community regarding sexual assault topics.
Receives referral information during agency business hours. Notifies clients and any community partners involved of appointments. Maintains client calendar for services. Verifies intake information and sets up records in EMR system.
Under the direction of the Program Coordinator, assists in onboarding new community and UF therapists and coordinating client services in house and referrals to community therapists.
Organizes regular team meetings with division employees and community therapists. Sends out agendas, handouts, and any other documentation needed for the meetings. Creates the in person or online invites, takes notes and distributes meeting notes after.
Serves as a subject matter expert for the EMR system, training new staff on best practices and policies and procedures regarding client notes and reporting functions.
Expected Salary:
$42,000 to $52,000 annually depending on education and experience.
Required Qualifications:
Bachelor's degree in an appropriate area; or an equivalent combination of education and experience.
Preferred:
Bachelor's degree in social work and four years of working with grant funding and reporting.
Valid driver's license and access to personal vehicle required.

Depending upon the department’s needs, interviewing shifts are available between 10AM and 10PM on weekdays, 12PM to 6PM on Saturdays, and 3PM to 6PM on Sundays.

Performs nutrient intake analysis, assesses nutritional needs of patients, develops and
implements nutritional care plans for Ryan White patients at UF CARES and within Ryan
White Jacksonville network.
Assesses learning needs and readiness of patients and families; provides individualized
age-appropriate nutrition information to patients and families, ensuring comprehension
and re-educating as needed.
Participates in discharge planning or patient care/case conferences; follows-up on
recommendations and care plans with team; coordinates and transitions care from

pertinent nutrition related programs as needed.
Provides for counseling of patients and/or their support network when special dietary
needs indicate that explanations and/or instructions are needed.
Consults and works with community resources, professional organizations and other

Documents nutrition information in the electronic medical record. Maintains all records
and reports.
Participates as a member of the healthcare team and acts as liaison between patients,
families and the clinical team to ensure quality customer service.
Assists with developing, updating, and revising of nutritional educational materials,
including annual review of dietary manual. Develops and presents health education and
promotion programs, including training workshops, conferences, and community
presentations.
Attends and participates in staffing, department and hospital-wide meetings
Expected Salary:
$52,000 to $65,000 annually, depending on education and experience.
Minimum Requirements:
Bachelor's degree in dietetics, foods and nutrition or food service administration, completion of American Dietetic Association approved dietetic education program. Licenses in Dietitian in accordance with Chapter 468, Florida Statutes.
Preferred Qualifications:
Two years clinical experience in a healthcare setting and/or with HIV/AIDS population preferred.
Master's degree in an appropriate area of specialization; or a bachelor's degree in an
appropriate area of specialization and two years of appropriate experience

Application Records & Compliance: • Maintain accurate and complete electronic records of pesticide and fertilizer applications, including product names, EPA registration numbers, fertilizer grades, application rates, dates, treated areas, methods, and applicator identification.• Ensure records are maintained in accordance with regulatory requirements and retained for the required period.• Support compliance documentation by organizing and maintaining accessible records as directed.Procurement & Inventory of Agricultural Supplies: • Assist with ordering, receiving, tracking, and inventory management of agricultural supplies, including pesticides, fertilizers, irrigation materials, plastic mulch, seeds, personal protective equipment (PPE), and related items.• Support proper handling, storage, and disposal of agricultural inputs in compliance with EPA, FDACS, OSHA, and institutional guidelines.• Communicate supply needs or inventory discrepancies to supervisory personnel.Essential and Other Duties: • Participate in natural disaster preparedness and recovery activities, including frost protection, hurricane preparation and cleanup, drought mitigation efforts, and storm-related response activities, as assigned.• Perform other related duties as assigned to support research and farm operations.
